For the times when students feel under the weather while at school, you can rest assured that our School Nurse is here to help. In order to provide the proper care, we ask parents to download and complete the following forms and return them to your child's teacher at the beginning of the school year. You may also be asked to fill out additional forms.

SDMS students also have access to a multitude of health professionals and services, including:
- Speech Pathologist
- School Psychologist
- Occupational Therapists
- Services for the Hearing Impaired
In order to keep all of our students healthy, we request that certain immunications be administered. You can download the School Immunization Requirements list from the Indiana State Department of Health here .
Please make sure your child eats something prior to receiving vaccinations.
A $3.00 donation per child is requested for all childhood vaccines. (No one is ever turned away for inability to donate.)
All recommended childhood vaccines are offered at our monthly childhood immunization clinics. An up-to-date shot record is required in order to administer immunizations to anyone. Vaccines are offered to ages 1 month thru 18 years at these clinics. All children under 18 years of age are required to have parent/guardian present.
We do not give vaccines to anyone 19 years or older at our childhood immunization clinics. If you have a high school student that is 19 or older, please contact us to inquire about vaccines that are needed, costs, availability, and appointment scheduling.
